RE: City -owned Backflow Device Testing
Dear Jeff,
Pursuant to Terms and Conditions Section IC of the Purchased Services Agreement No. 086-20
(2020 Contract), please be advised that the City wishes to extend the duration of said Contract to
September 30, 2023. As mutual agreement is required to extend the contract duration pursuant to the
current rates established within Exhibit A, please affirm your concurrence by signing below.
Also, per Section 12 of the contract, please submit a current copy of the Certificate of Liability
insurance with endorsements that show the City listed as additional insured.
