Total Contract Approved: Amount shall reset to reflect new Council -approved contract totals authorized prior to the requested change order. $50,000-$100,000 require Mayoral Approval. $1O0,000 and over require Council Approval. Works Director )ehis Ryan Printed Name Mayor City Clerk Council Approval Date 9 SKILLINGS EXHIBIT A SCOPE OF WORK SUPPLEMENT Prepared for: CITY OF PORT ORCHARD SUPPLEMENTAL AGREEMENT NO. 1 25164 — RUBY CREEK CULVERT REPLACEMENT June 2, 2026 Introduction: The prime Scope of Work assumed that only Minimum Requirements 1- 5 would be triggered due to the limited proposed impervious surface. Site design has evolved and the proposed new and replaced impervious surface will exceed 5,000 square feet, requiring all minimum requirements. Additionally, frontage improvements connecting to existing improvements and extending outside of the stream buffer were requested by the City. This supplemental scope is to describe the additional effort associated with expansion of the design footprint and includes additional topographic survey, additional design sheets, specifications, and additional stormwater design including reporting. The prime contract included a Management Reserve Fund of $30,000, which will be utilized as budget for the additional scope. Project Assumptions: • Skillings will provide Field and Office Support to provide an updated Topographic Survey map. • A full drainage report addressing all Minimal Requirements will be required. • Treatment and flow control will not be required at the TDA level. • Stormwater from two identified existing off -site outfalls in the northeastern and southwestern areas will be rerouted as needed to accommodate the proposed sidewalk and associated improvements. The northeastern outfall is from a storm sewer system, and the southwestern outfall is a roadside conveyance ditch. Both are excluded from and will not receive stormwater treatment under this project. • Frontage improvements will be limited to the area between existing frontage improvements on the east side of Sidney RD SE and will be extended outside of the stream buffer on the west side of Sidney RD SE. • Frontage improvements shall be comprised of the addition of a 6' bike lane, curb and gutter, and 8' cement concrete sidewalk. • Widths of frontage improvements will be tapered to connect to existing improvements where connected. Task Description: • Project Management and Administration. • QA/QC and Sign Topographic Map Task 2 — Field Mapping and Drafting Assumptions: • Redeploy Field Crew to the site • Additional Mapping as per the Exhibit sketch. • Assuming that the Survey Control that was previously set is not disturbed. • Field data and pictures will be uploaded. Task Description: 1. Field Data Collection 2. Process Field Data, Draft and Final update to topographic map. Deliverables: • PDF File of the combined efforts for the Topographic Mapping — provided via Email. • Civil 3D Cad files — to be provided to the City— via Email. Task 3 — Survey Control Replacement (as needed) Assumptions: • Survey Control Replacement will occur, if needed. Task Description: 1. Establish new survey control. 2. QA/QC of new survey control.
