Scope of Work:
The Sewer Lift Station Rehabilitation project upgrades control panels at six lift stations and installs a
generator at one lift station.

Describe the progress made on this project to date, the current status of the project, any barriers to
completion, and the reason(s) for the extension request.
There has been an unforeseen production delay in the construction of the control panels. This is the
reason for the extension request. The factory acceptance testing has been completed on 4 of the 6
control panels for the project and are currently in transit to the sites. The GC is in constant
communication with the Hardware System Integrator and Software Systems Integrator that are
building the remaining panels.
The generator has been installed and tested. The roof structures for the panels have been completed.
The remaining items for the project are the installation and programming of the 6 control panels.
By signing this form, I acknowledge the following statements:
1. I am a representative of the Jurisdiction identified above who is authorized to request this
change to the Funding Agreement between the Jurisdiction and the Public Works Board (PWB).
2. I have reviewed the PWB's Project Completion and Hold Back Policy and understand the PWB's
definition of "project completion".
3. The project is scheduled to be complete by the requested new time of performance end date
identified above.
4. Regardless of whether the PWB approves this request or not, if the project is not complete by
the time of performance end date as defined in the funding agreement or any amendment(s),
then the PWB may find the agreement to be in default.
5. If the PWB approves this request, the PWB will use the information provided in this form to
prepare an amendment to the funding agreement.
