CONTRACT AMENDMENT
This CONTRACT AMENDMENT is made and entered into between KITSAP COUNTY,
a municipal corporation, with its principal offices at 614 Division Street, Port Orchard,
Washington 98366, hereinafter "COUNTY", and the City of Port Orchard, a municipal
corporation having its principal offices at 216 Prospect Street, Port Orchard, Washington
983667, hereinafter "CITY".
In consideration of the terms and conditions contained herein, the parties agree
that their Contract, numbered as Kitsap County Contract No. KC -509-24, and executed on
March 12, 2025, shall be amended as follows:
1. The "Request and Agreement for Work, Equipment, and/or Materials" form attached as
Attachment A to the original Agreement contains Information that was inadvertently
pre -populated in the "Estimate Attached," "Project Completion Date," and "Total Cost
Not to Exceed" fields. This amendment revises Attachment A by removing the pre -
populated information from these fields and restoring them as blank fields. The revised
form will allow future work requests, project completion dates, cost limitations, and
other project -specific information to be accurately documented and mutually agreed
upon at the time each request is submitted.
2. Except as expressly provided in this Contract Amendment, alt other terms and
conditions of the original Contract, and any subsequent amendments, addenda or
modifications thereto, remain in full force and effect.
This amendment shall be effective upon execution by the Parties.
