*This Change Order has been updated to reflect scrivener's error. Pacific Air's proposal listed two options for fixing the AH-
3 serving the 3`d floor — one during business hours and one after business hours/weekend. We inadvertently added both
options versus the one chosen. Change Order i — 2018 amount has been decreased by $3883.67.

While performing the regularly scheduled maintenance, our technician noted the
following:
AH-1 sin 042A serving the Vt Floor
The cold water inlet actuator on this unit has failed. We recommend replacing the cold
water inlet actuator for proper operation of this unit.
Cost of the above: $ 1,386.00 plus tax
B-2 sin 7-99W serving the Entire Building
The exhaust stack on this unit drips water. We recommend sealing the exhaust stack
with high temperature caulking and extend the drain port to the floor drain with tubing.
Cost of the above: $ 617.00 plus tax
Once the above work has been completed, the units will be tested for proper operation.
If further repairs are necessary, we will contact you.

The three way water mixing valve on this unit has failed. We recommend replacing the valve,
actuator, and copper fittings for proper operation of this unit.
