A RESOLUTION OF THE CITY OF PORT ORCHARD,
WASHINGTON, AUTHORIZING THE MAYOR TO EXECUTE
CONTRACT WITH THE DOCTORS CLINIC FOR CDL/DOT EXAM
AND CERTIFICATIONS
WHEREAS, on January 14, 2015, the Doctors Clinic entered into a contract with the City
of Port Orchard (the "City") for the DOT/CDL exam and certifications for $103 per exam, which
include basic hearing and vision testing for the exam, required vital signs and the required
urinalysis; and
WHEREAS, this contract will now expire on December 31, 2016, at which time the City
of Port Orchard will again seek proposals from local physicians or physician clinics that are
desirous of becoming the City's contact for employee DOT/CDL certifications and exam; and
WHEREAS, According to the Public Works Union Contract, Article 8, section 8.7, "The
Employer shall reimburse the employee for the renewal fee of the (Commercial Driver's License)
CDL endorsement and the DOT physical examination at the City's bid doctors; now, therefore;
THE CITY COUNCIL OF THE CITY OF PORT ORCHARD, WASHINGTON, HEREBY RESOLVES
AS FOLLOWS:
THAT: The Mayor is authorized to execute a contract with The Doctors Clinic,
PC for CDL/DOT exam and certifications for $103 per exam to expire on
December 31, 2016.
PASSED by the City Council of the City of Port Orchard, SIGNED by the Mayor and attested
by the Clerk in authentication of such passage this 26th day of January 2016.
