MAYOR TO EXECUTE CONTRACT EXTENSION
WITH THE DOCTORS CLINIC FOR CDL/DOT

WHEREAS, on January 4, 2008, the Doctors Clinic entered into a contract with
the City of Port Orchard (the "City") for the DOT /CDL exam and certifications for $78
per exam, which include basic hearing and vision testing for the exam, required vital
signs and the required urinalysis; and
WHEREAS, this contract will now expire on December 31, 2009, at which time
the City of Port Orchard will again seek proposals from local physicians or physician
clinics that are desirous of becoming the City's contact for employee DOT/COL
certifications and exam; and
WHEREAS, According to the Public Works Union Contract, Article 8, section
8.7, "The Employer shall reimburse the employee for the renewal fee of the (Combined
Drivers License) COL endorsement and the DOT physical examination at the City's bid
doctor;" now, therefore;
THE CITY COUNCIL OF THE CITY OF PORT ORCHARD,
WASHINGTON, HEREBYRESOLVESAS FOLLOWS:
THAT: The Mayor is authorized to execute a contract extension with
the Doctors Clinic for COL/DOT exam and certifications for $78 per
exam to expire on December 31, 2009.
