A RESOLUTION OF THE CITY OF PORT ORCHARD,
WASHINGTON, AUTHORIZING THE MAYOR TO EXECUTE
CONTRACI' NO. C005-10, WITH THE DOCI'ORS CLINIC FOR
CDL/DOT EXAM AND CERTIFICATIONS
WHEREAS, on April 14, 2009, the Doctors Clinic entered into a contract with
the City of Port Orchard (the "City") for the DOT/CDL exam and certifications for $78
per exam, which include basic hearing and vision testing for the exam, required vital
signs and the required urinalysis, of which is being increased by $2.00 for a fee of $8o;
and
WHEREAS, this contract will now expire on December 31, 2010, at which time
the City of Port Orchard sought proposals from local physicians or physician clinics that
are desirous of becoming the City's contact for employee DOT/CDL certifications and
exam; and
WHEREAS, According to the Public Works Union Contract, Article 8, section
8.7, "The Employer shall reimburse the employee for the renewal fee of the (Combined
Drivers License) CDL endorsement and the DOT physical examination at the City's bid
doctor;" now, therefore;
THE CITY COUNCIL OF THE CTIY OF PORT ORCHARD,
WASHINGTON, HEREBY RESOLVES AS FOLLOWS:
THAT: The Mayor is authorized to execute Contract No. Coos-to , with
the Doctors Clinic for CDL/DOT exam and certifications for $8o per
exam to expire on December 31, 2010 .
PASSED by the City Council of the City of Port Orchard, SIGNED by the Mayor and
attested by the Clerk in authentication of such passage this 22nd of Decemb oog.
