THE CITY COUNCIL OF THE CITY OF PORT ORCHARD
DOES HEREBY RESOLVE:
THAT: The Mayor of the City of Port Orchard
be and hereby is authorized to sign Agreement between
Hos Bros. Construction, Inc. and City of Port Orchard
relative to Port Orchard/KCSD#5 Wastewater Facility
Improvements Schedule B-Force Main as attached.
PASSED by the City Council and APPROVED by
the Mayor this 13th day of February, 1984.

Schedule B - Force Main
SUBJECT: Agreement between Hos Bros. Construction, Inc. and
City of Port Orchard
Gentlemen:
As a result of the inspection of the above referenced project on February
3, 1984, and in accordance with the provisions of the subject agreement,
this firm recommends to the City of Port Orchard that the project be
accepted.
As set forth in the Contract General Conditions, your responsibility
ceases upon Owner acceptance of the work, except as provided in Section 17
"Guaranty," and as provided in the performance Bond.
Release of retainage and final payment will be executed in accordance with
RCW Chapter 60.28. The following documents are necessary before the
retainage can be released:
1. Final Certificate of Payment (Final Retainage).
2. Release of liens from material and labor suppliers.
3. Certified statement of nature and source of offshore items.
4. Contractor's.Affadavit of Payment of Debts and Claims. (enclosed)
5. Labor Standards Certificate.
6. Consent of Surety for release of retainage.

Please submit the above documents as soon as possible so that this project
may be finalized.
